About the Role

We are seeking a highly motivated and experienced Community Mental Health Nurse to join our team at Cornwall Partnership NHS Foundation Trust. As a key member of our multidisciplinary team, you will be responsible for delivering high-quality care to individuals experiencing severe and enduring mental health problems.

Key Responsibilities
  • Assess and manage a defined caseload of patients, providing comprehensive care and support to promote recovery and independence.
  • Develop and implement effective care plans, working collaboratively with patients, families, and other healthcare professionals to achieve optimal outcomes.
  • Contribute to the development and implementation of innovative health and social care services, ensuring a seamless interface between primary and secondary care.
  • Provide clinical leadership and supervision to staff, promoting a culture of excellence and continuous improvement.
  • Participate in service development and quality improvement initiatives, ensuring that our services are responsive to the needs of our patients and communities.

Person Specification
  • Current professional registration as a Registered Mental Health Nurse (RMN)
  • Post-basic training in mental health nursing
  • Extensive experience of working with mental health patients
  • Ability to demonstrate clinical leadership and supervision skills
