Live-In Support Coordinator

Estimated Salary: £38,000 - £42,000 per annum

We are looking for a highly organized and motivated individual to join our team as a Live-In Support Coordinator. As a key member of our team, you will be responsible for providing support and guidance to our care workers in the field.

The ideal candidate will have excellent communication and interpersonal skills, with the ability to work independently and as part of a team. You will be responsible for creating person-centered care plans and risk assessments, reviewing packages of care, and auditing communication logs and medication charts.

About the Role:

  • Provide support and guidance to care workers in the field
  • Create person-centered care plans and risk assessments using our computerized care planning system
  • Review packages of care and make adjustments as needed to ensure service users' needs are met
  • Audit communication logs and medication charts to ensure accuracy and quality

Requirements:

  • Proven experience in a care support role
  • Strong organizational and time management skills
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
