Personalised Cancer Care Transformation Lead

This is an exciting opportunity for an experienced, dynamic, and enthusiastic Therapist to lead transformation work across South Tees NHS Trust within the existing cancer pathways, to consider how prehabilitation can be accessed and delivered to improve the outcomes of patients following a cancer diagnosis.

The post holder will be employed as a Senior Specialist Therapist and will lead the project at South Tees Hospitals NHS Foundation Trust for 12 months.

The project will explore the feasibility of developing and delivering a model of prehabilitation. It will aim to use or adapt existing services delivered in the acute trust and by the community teams to support the psychological wellbeing, dietary and physical activity needs of newly diagnosed cancer patients. The ultimate aim is to optimise patients' health and wellbeing before, during and after primary cancer treatment.

Main Responsibilities

The postholder will be required to scope existing services that can be accessed within primary and secondary care by cancer patients with the aim to develop a model of prehabilitation for cancer. The post holder will support clinical teams to explore tumour specific prehabilitation interventions where required.

The post will seek to make a positive difference to those individuals diagnosed with cancer to ensure they have a good experience of cancer care and support them to live as healthily and happily as possible.

The post holder will work closely with cancer services and clinical teams, ICB, Macmillan and the Northern Cancer Alliance. They will also be responsible for ensuring that effective and close collaborative working relationships are established and maintained with health service and social colleagues at all levels.

This post offers development in clinical and leadership skills and the post holder will be supported to attend relevant post graduate courses. We have an established and comprehensive training programme and our staff are encouraged and supported in their CPD through workshops, clinical supervision and the appraisal process. There is also an expectation to actively participate in audit and research and the Hospital has close links with both the University of Teesside and Durham University.
