Care Coordinator

Cambridgeshire and Peterborough NHS Foundation Trust is seeking a skilled Care Coordinator to join our team. With an estimated salary of £35,000 per annum, this role offers a challenging and rewarding opportunity to work in a dynamic environment.
About the RoleThe successful candidate will be responsible for assessing patient progress, evaluating therapy interventions, and promoting independence. They will also need to communicate effectively with patients, carers, and colleagues, using tact and persuasive skills.
This role requires strong organizational skills, good general education up to GCSE equivalent, and evidence of relevant short-course and in-service training. A full UK driving license and use of a vehicle are essential, as you will be required to travel independently around the county.
Key Responsibilities- Assess patient progress and evaluate therapy interventions.
- Promote independence and prevent hospital admissions.
- Effectively communicate with patients, carers, and colleagues.
- NVQ Level 3 or equivalent level of knowledge and experience.
- Willingness to undertake training to meet competency framework.
- Full UK driving license and use of a vehicle.
