Key Responsibilities
  • Deliver packages of care to patients in their own homes, including residential homes and nursing homes.
  • Work collaboratively with our multidisciplinary team to ensure seamless care coordination and delivery.
  • Develop and maintain strong relationships with patients, families, and carers to ensure their needs are met and exceeded.
  • Contribute to the development and implementation of care plans that promote patient independence and well-being.
  • Participate in regular team meetings and contribute to the improvement of our services.
Requirements
  • NVQ Level 3 qualification in care or equivalent experience.
  • Evidence of experience in a care setting.
  • Hold a valid driving license.
