About the Role

We are seeking a skilled Community Support Worker to join our team at Cornwall Partnership NHS Foundation Trust. As a Community Support Worker, you will play a vital role in delivering high-quality care to patients in their own homes, as well as in residential and nursing homes.

Main Responsibilities

As a Community Support Worker, your key responsibilities will include:

  • Delivering packages of care to patients in their own homes, as well as in residential and nursing homes
  • Working flexibly to meet the needs of the service and being rostered to provide the required service
  • Providing a versatile and responsive service to ensure that opportunities for patients' reablement are optimised within the community setting

We deliver community and hospital-based care to improve people's physical and mental health. We also provide specialist support to people with dementia or a learning disability.

Person Specification

To be successful in this role, you will need to have:

  • NVQ Level 3 qualification in care or equivalent experience
  • Evidence of experience in a care setting
  • A valid driving licence
