We are seeking a skilled and compassionate Nurse Assessor to join our Cornwall Onward Care Team, based in Derriford Hospital. The successful candidate will play a vital role in supporting patient flow from an acute hospital, working closely with an experienced team to achieve a collaborative approach to hospital discharge.

The ideal candidate will have excellent communication skills, ability to work on their own initiative, and cope with changing workload/pressures in a demanding environment. They will also have a professional registration - NMC, professional qualification - RN, RMN, and post graduate learning - Alert or Long term conditions.

The successful candidate will contribute to follow up assessments, care planning, identification of resources and service delivery to facilitate the appropriate and timely discharge of patients. They will also work flexibly and proactively in response to the needs of the service, working across organisational boundaries.
