About the Role

We are seeking a highly skilled and experienced Care Coordinator to join our team at Greater Middlesbrough Primary Care Network (PCN). As a key member of our multidisciplinary team, you will play a vital role in supporting the delivery of high-quality patient care.

Key Responsibilities

  • Prioritise workload appropriately and in accordance with Practice, local and national guidelines.
  • Perform general administration duties, including QOF management and recall in line with meeting targets for patient care.
  • Develop awareness of IIF targets and LIS, and make submissions in CQRS for monthly/quarterly claims.
  • Conduct ad hoc searches for audit purposes when requested, and lead on the implementation of new IT projects.
  • Organise and attend meetings, such as GSF, and maintain minutes or updates to the GSF spreadsheet.
  • Send text messages to patients using the Accurx messaging system, and review and update the Practice website to ensure information is accurate and up to date.
  • Manage workflow and document management, including scanning and coding medical information, and process Subject Access Request's and Insurance reports.

Greater Middlesbrough PCN is a group of 8 GP Practices with a weighted list size of 70,192 patients. We are committed to developing all staff with a loyal extended primary healthcare team.

Requirements

  • Previous NHS experience and a high standard of IT skills, including knowledge of clinical systems, SystemOne.
  • Good understanding of Medical Terminology and ability to give attention to detail and follow protocol to ensure accuracy is maintained to a high standard.
  • Ability to work on own initiative, recognise confidentiality and sensitivity of issues, and handle accordingly.
  • Good interpersonal skills to work effectively as part of the Practice team.
