Adult Nurse Manager
About the Role:
We are a well-established and forward-thinking provider of complex care packages within the community, specialising in adult complex care. This role is based in South East London where we have secured further packages during an exciting growth phase.
Responsibilities:
- Provide face-to-face support to adult clients and their families, whilst creating personalised care plans for each individual.
- Work closely with a core team of carers, training and supporting them with their clinical skills and activities.
- Design and deliver tailored care plans specific to an individual's needs.
- Effectively manage the multi-disciplinary team through strong communication and teamwork.
- Provide ongoing monitoring, support, and assessment of clients' physical and physiological needs within your caseload.
- Deliver nursing interventions, including the management of medicine delivery.
- Build and maintain sophisticated relationships with clients' families.
- Work closely with community teams, CHC nurses, and CCG commissioners to establish and maintain bespoke packages of care.
Requirements:
- RGN with an active NMC PIN.
- Supportive management style that can bring out the best in others.
- Up-to-date experience of tracheostomy care and ventilators within the last 2 years.
- Advanced communication skills verbally and written to liaise effectively with patients, families, and the MDT.
- Suitable for Band 5 / Band 6 Nurses with evidence of effective leadership and management of HCAs.
- Full UK Driving Licence desirable but not essential.
Benefits:
- Up to £45,000 per annum DOE.
- 1 day's working from home per week / 4 days in the community.
