We are seeking a compassionate and experienced Community Health Worker to join our Mental Health Accommodation Service at Leeds City Council. In this dynamic role, you will work closely with individuals transitioning from acute mental health hospital wards, prison, homelessness, etc., to community-based mainstream accommodation.

The estimated salary for this position is £35,000 - £45,000 per annum, depending on experience.

In this critical role, you will:

  • Deliver holistic care packages tailored to individual needs and preferences.
  • Establish and maintain strong relationships with service users, families, and community partners to ensure seamless support and advocacy.
  • Develop and implement effective care plans that address physical, emotional, and social needs.
  • Participate in interagency collaborations to promote coordinated care and resource allocation.

To thrive in this position, you will require:

  • Strong clinical knowledge and skills in areas such as psychology, psychiatry, and social work.
  • Experience working with marginalized populations, including those with complex needs and vulnerabilities.
  • NVQL3 in Health and Social Care or an equivalent qualification and a commitment to ongoing professional development.
