Primary Care Diabetes Team Member

Diabetes Nurse Specialist Job Description: We are seeking an experienced Diabetes Nurse Specialist to join our team. As a key member of our primary care Diabetes team, you will be responsible for managing patient caseloads, conducting regular reviews and providing education on optimal diabetes management.
Main Accountabilities:
- Provide high-quality care to patients with Type 2 diabetes, including routine check-ups, medication management and education
- Collaborate with healthcare professionals to ensure seamless care transitions and effective communication
- Participate in quality improvement initiatives to enhance patient outcomes and team performance
- Stay up-to-date with the latest clinical guidelines and best practices in diabetes management
Requirements: To be successful in this role, you will require a strong background in managing diabetes within primary care, including experience with optimising oral hypoglycaemics and conducting point-of-care testing. You must also possess excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
