We are seeking an experienced Community Health Practitioner to join our team in Swindon. This rewarding role involves providing high-quality nursing care to patients in their own homes or community settings, promoting independence and achieving healthcare goals.

About the Role:
  • This position is based in Swindon and offers a competitive salary of £28,000 - £34,000 per annum.
  • You will work autonomously in patients homes and community settings, ensuring safe and effective care delivery.
  • Manage a daily caseload, prioritising tasks and responding to changes in patients conditions.
  • Use digital tools and systems to plan, record, and report on care delivery in line with organisational policies.
Key Responsibilities:
  • Clinical Care
    • Deliver a range of nursing interventions, including wound care, medication administration, catheter care, and end-of-life care.
    • Conduct holistic assessments of patients physical, emotional, and social needs to develop and implement individualised care plans.
    • Monitor and evaluate patients progress, adapting care plans as necessary to achieve the best outcomes.
    • Provide health education and advice to patients and carers to support self-management and promote independence.
  • Team Collaboration
    • Work closely with other healthcare professionals, such as GPs, therapists, social workers, and voluntary sector partners, to ensure coordinated care.
    • Participate in multidisciplinary team meetings and case conferences to support care planning and review.
    • Act as a patient advocate, ensuring their preferences and needs are central to care planning and decision-making.
