Personalized Support Specialist

We are seeking a compassionate and skilled Personalized Support Specialist to join our HC-One team. As a Senior Care Assistant, you will play a vital role in providing high-quality care to our residents, focusing on their individual needs and preferences.

About the Role

In this rewarding position, you will be responsible for developing and implementing personalized care plans for our residents. You will work closely with our Registered Nurse and Care Manager to deliver excellent care and support to our residents. Additionally, you will be responsible for leading a team of Care Assistants, promoting a culture of excellence and innovation within the team.

Key Responsibilities
  • Develop and implement personalized care plans for residents
  • Lead a team of Care Assistants and promote a culture of excellence
