Care Services Practitioner
Lancashire County Council is recruiting for a Care Services Practitioner to join their team in the Children Leaving Care Service. The successful candidate will work as part of a dedicated team to ensure the development, delivery, and continuous improvement of services for children, young people, and their families and carers in Lancashire.

- Children and young people achieve their full potential in education, learning, and future employment.
- Children and young people enjoy healthy lifestyles and know how to help others.
- Children, young people, and families have a voice in shaping the support they receive.
- Children and young people live in Lancashire where they can enjoy a good quality of life, be happy, and want to stay.
To deliver these objectives, the post holder will:
- Work as part of the Leaving Care Service to ensure the development, delivery, and continuous improvement of services for children, young people, and their families and carers in Lancashire.
- Work in partnership with colleagues within Children's Social Care to ensure consistency of practice across the county and promote the development of a supportive collaborative culture across the service.
- Work in partnership with children and young people, parents, and carers, education, health, and other agencies/professionals to provide effective support that improves outcomes for children, young people, and their families.
- Ensure young people leaving care are at the centre of all practice and processes.
- Ensure children and young people are provided with the right support at the right time.
- Ensure the effective and efficient use of available resources in support of the well-being and development of children and young people leaving care.
