About the Role

  • You will work closely with colleagues to develop and implement individualized care plans, ensuring seamless transitions and optimal outcomes.
Key Responsibilities
  • To deliver comprehensive nursing assessments, identifying key areas of need and developing targeted interventions.
  • To collaborate with healthcare professionals and community stakeholders to ensure cohesive care delivery.
  • To contribute to service development, sharing expertise and best practices to drive continuous improvement.

Salary: £32,000 - £43,041 per annum (dependent on experience)
