About the RoleThis is an exciting opportunity to work in a pioneering Nurse-led approach to patient care in the community, with self-managed nursing teams. As a Senior Practitioner District Nurse, you will be responsible for providing high-quality nursing care and health promotion to patients with complex, high-risk, and supported/self-care conditions.
Key Responsibilities- Undertake holistic assessments, planning, implementation, and evaluation of care, including Chronic Disease Management, Tissue viability, Palliative Care, Nurse Prescribing, Extended and Supplementary Prescribing.
- Provide first contact assessment, care, and referral.
- Deliver continuing care, rehabilitation, and National Service Frameworks.
- Develop and implement public health and health promotional programmes to improve health and reduce inequalities.
As a Senior Practitioner District Nurse at Guy's and St Thomas' NHS Foundation Trust, you will have the opportunity to further develop and enhance your skills with the support and training available by working alongside experienced nurses and specialists. You will also have access to training and mentorship programs, financial, health, and wellbeing support.
RequirementsTo be successful in this role, you will need to be a registered nurse with a strong clinical background and excellent communication skills. You will also need to be able to work collaboratively with other healthcare professionals and statutory and voluntary organisations.
Working ArrangementsThis is a full-time role working on a 7-day shift service, with hours between 8am and 8pm. You will be required to work collaboratively with your team and other healthcare professionals to ensure all patients receive high-quality care.
Education and DevelopmentWe are committed to the ongoing development of our staff and offer a range of learning opportunities, including clinical supervision, performance appraisal, and mandatory training.
Service DevelopmentWe are looking for a Senior Practitioner District Nurse who is passionate about service development and is able to work across the organisation to support and promote Neighbourhood Nursing.
To adhere to the Nursing and Midwifery Council [NMC] Code of Professional Conduct and be conversant with other relevant advisory papers.
Responsible for providing day-to-day co-ordination of workload in your own team and network.
Review and agree staffing levels, including Bank and Agency requirements ensuring that rostering is efficient and effective and meets the needs of the service.
Informal performance management and management of behaviours and conduct as a team.
Undertake as a team the process of continuous performance review.
Respond to complaints and incidences as required and undertake any relevant investigations.
Actively implement learning from complaints/incidents in your team and the wider network.
Ensure Health and Safety and Risk requirements are met.
Assist in monitoring the quality of the Neighbourhood Nursing Service.
Contribute to the development of quality measures.
Attend and participate in locality meetings with other teams as well as wider meetings across Health and Social Care.
Aware of and work with GSTTs' Policies and Procedures.
