District Nurse Practitioner

We are seeking a highly skilled and experienced District Nurse Practitioner to join our Complex Needs Team at East London NHS Foundation Trust. The successful candidate will be responsible for providing high-quality nursing care to patients with complex needs, working closely with other healthcare professionals to ensure comprehensive and coordinated care.
As a District Nurse Practitioner, you will be responsible for assessing, planning, implementing, and evaluating the care of patients with complex needs, including those with long-term conditions, disabilities, and mental health conditions. You will work in partnership with patients and their families to identify their needs and preferences, and to develop and implement individualized care plans that promote independence and well-being.
Key Responsibilities:
Assessment and Care Planning:
Conduct comprehensive assessments of patients with complex needs, including physical, emotional, and social assessments.
Develop and implement individualized care plans that are tailored to the needs and preferences of each patient.
Care Delivery:
Deliver high-quality nursing care to patients with complex needs, including those with long-term conditions, disabilities, and mental health conditions.
Work in partnership with patients and their families to identify their needs and preferences, and to develop and implement individualized care plans.
Collaboration and Communication:
Work closely with other healthcare professionals, including doctors, therapists, and social workers, to ensure comprehensive and coordinated care.
Communicate effectively with patients, families, and healthcare professionals to ensure that care needs are met and that patients receive the best possible care.
Professional Development:
Maintain and develop your clinical skills and knowledge through ongoing training and professional development.
Contribute to the development of policies and procedures that promote high-quality care and patient safety.
Participate in quality improvement initiatives to ensure that care is delivered in accordance with best practice.
