Job Title: Advanced Practitioner - Specialist Clinical Pharmacist

About the Role

We are seeking an experienced Clinical Pharmacist to join our team as an Advanced Practitioner - Specialist Clinical Pharmacist. As a key member of our Acute Medical Directorate, you will be responsible for providing specialist clinical pharmacy and medicines optimisation services to patients in the Emergency Department and Acute Medical Unit.

Key Responsibilities
  • Assess patients and identify medicine-related causes for attendance and admission
  • Develop treatment plans to optimise care and improve patient outcomes
  • Provide specialist clinical pharmacy and medicines optimisation services to patients and healthcare professionals
  • Conduct risk management for the pharmacy service and ensure compliance with medicines legislation and national guidelines
  • Contribute to the development and implementation of clinical pharmacy services and policies
Requirements
  • Professional knowledge acquired through a vocational degree in pharmacy
  • Currently registered as a pharmacist in the UK
  • Clinical specialist knowledge acquired through postgraduate level training
  • Experience in influencing and persuading senior clinicians in relation to optimal medicine treatment for patients
  • Experience in reviewing medication errors and analysing trends
  • Experience in providing an on-call or resident pharmacy service
