Advanced Clinical Practitioner

About the Role:
We are seeking an experienced Advanced Clinical Practitioner to join our team in providing high-quality urgent care services to patients in need. As a key member of our team, you will be responsible for assessing and treating patients with undifferentiated conditions in a General Practice setting.
Main Responsibilities:
- Assess and treat patients face-to-face or via telephone triage
- Work in a flexible environment, choosing between Primary Care Centres or home visiting shifts
- Collaborate with a skilled mix of GPs, Advanced Nurse/Clinical Practitioners, and Paramedic Practitioners, supported by an experienced operational team
- Have access to basic daytime GP notes via GP Connect, summary care records, special notes, and child protection information sharing
- Use electronic prescribing systems and work in a clinical and managerial capacity
