  • Key Responsibilities:

  • Manage long-term conditions, such as diabetes, hypertension, and asthma, ensuring effective treatment plans and regular follow-ups.
  • Collaborate with other healthcare professionals to provide seamless patient care and achieve optimal health outcomes.
