We are seeking a highly motivated and innovative Care Coordinator to join our Ealing Community Rehabilitation Team (psychosis pathway). As a key member of our multi-disciplinary team, you will be responsible for delivering individualized and recovery-focused care to individuals with complex care needs related to psychosis and co-existing substance use and physical health problems.
Main Responsibilities- Assess, plan, implement, and evaluate comprehensive care packages for clients with severe and enduring mental health problems.
- Develop and deliver psychosocial interventions and support programs for clients and their families.
- Collaborate with multidisciplinary teams to provide intensive support and rehabilitation in the community.
- Conduct risk assessments and develop strategies for managing risk.
- Work with clients who are difficult to engage and develop strategies for engagement.
- Current professional qualification in Mental Health Nursing/Social Work/OT.
- Registration with a professional body such as NMC or other relevant body.
- Evidence of post-registration training and CPD.
- Experience of working in multi-disciplinary mental health teams with clients with severe and enduring mental health problems.
- Knowledge of CPA process and current legislation and guidance applicable to this client group.
