We are seeking a highly skilled and experienced Senior Community Clinic Nurse to join our team at Oxleas NHS Foundation Trust. As a Senior Community Clinic Nurse, you will play a key role in providing high-quality care to individuals with learning disabilities, working closely with our multidisciplinary teams to ensure seamless care delivery.
Key Responsibilities- Lead the development of complex physical health practice for individuals with learning disabilities, promoting evidence-based care and clinical excellence.
- Provide specialist assessment, intervention, and management, working collaboratively with colleagues, service users, families, and other agencies.
- Develop and implement health promotion strategies to improve the overall health and wellbeing of service users.
- Liaise with multi-disciplinary colleagues and other professionals/agencies to address issues that create barriers to social well-being.
- Facilitate access to mainstream health care for individuals with learning disabilities, ensuring they receive the care they need.
- Registered Nurse (RNLD) qualification
- Ongoing professional development plus further specialist training to masters equivalent level
- Extensive post-qualification experience, providing adult learning disability services
- Supervisory roles with staff and students
- Experience of management or similar senior clinical experience
