Senior Community Nurse for Children with Disabilities

Oxleas NHS Foundation Trust is seeking a highly skilled and experienced Senior Community Nurse to join our team. As a Senior Community Nurse, you will play a key role in providing high-quality care to children with disabilities and their families.
Key Responsibilities:- Provide visible, high-calibre nurse leadership and support around the 'Complex Physical Health' (CPH) of Learning Disability within a multi-disciplinary framework.
- Lead the development of complex physical health of Learning Disability practice, promoting the clinical and professional development of staff within the community team, to ensure they provide holistic and evidence-based care.
- Provide specialist assessment, intervention, and management, working collaboratively with colleagues, service users, families, and other agencies.
- Lead on providing robust health liaison to acute and primary care to ensure our service users are able to access mainstream health services.
- Contribute to the continuing development of the ALD Service Physical Health Group.
- Apply national strategies, frameworks, and plans to identifying and meeting the needs of service users in relation to their physical well-being.

We manage hospital sites, including Queen Mary's Hospital, Sidcup, and Memorial Hospital, Woolwich, as well as the Bracton Centre, our medium secure unit for people with mental health needs.

As a Senior Community Nurse, you will be responsible for applying evidence-based care to support physical well-being, utilising LD-specific and mainstream health targets. You will assess and manage complex clinical issues in relation to service users and advise and collaborate with carers, professionals, and other agencies to offer support around complex clinical interventions and outcomes.
You will provide and delegate care delivery to ensure that the health needs of people with learning disabilities are met. You will identify and provide appropriate support to the social well-being of individual service users and address barriers to social well-being as part of the holistic health assessment and intervention.
You will liaise with multi-disciplinary colleagues and other professionals/agencies in order to address issues that create barriers to social well-being and report on unmet need and obstacles to senior colleagues.
You will employ health promotion techniques to develop the overall health and wellbeing of service users and prevent avoidable illness. You will identify resources to enable staff to deliver specific physical and mental health promotion interventions and develop training for staff, users, and carers around health promotion.
You will facilitate service users and carers in overcoming barriers to accessing primary care and other NHS services and facilitate access to mainstream health care, for the majority of people with learning disabilities.
You will provide intensive health care or other support to people with learning disabilities who have complex needs or who pose challenges to services. You will devise protocols and guidance around the provision of care to those with complex physical and or mental health needs or for those who challenge services.
