Job Title: Support Coordinator
Job Summary:
We are seeking a highly skilled and compassionate Support Coordinator to join our team at NFP People Careers. As a Support Coordinator, you will play a vital role in supporting stroke survivors and their families, providing emotional and practical support to help them recover and rebuild their lives.
Key Responsibilities:
- Provide emotional support and guidance to stroke survivors and their families
- Develop and implement personalized support plans to meet the unique needs of each individual
- Collaborate with healthcare professionals and other stakeholders to ensure comprehensive care
- Conduct regular home visits to assess the needs of stroke survivors and provide support
- Maintain accurate records and reports to track progress and identify areas for improvement

- Ability to work independently and as part of a team
- Strong organizational and time management skills
- Compassion and empathy for stroke survivors and their families
