Job Title: Support Coordinator

Job Summary:

As a Support Coordinator, you will play a vital role in supporting stroke survivors and their families, providing emotional and practical support to help them recover and rebuild their lives.

Key Responsibilities:

  • Provide emotional support and guidance to stroke survivors and their families
  • Develop and implement personalized support plans to meet the unique needs of each individual
  • Collaborate with healthcare professionals and other stakeholders to ensure comprehensive care
  • Conduct regular home visits to assess the needs of stroke survivors and provide support
  • Maintain accurate records and reports to track progress and identify areas for improvement
