Job Title: Senior Practitioner in Learning Disability and Autism Services

We are seeking a highly motivated and experienced individual to join our team as a Senior Practitioner in Learning Disability and Autism Services.

About the Role

This is an exciting opportunity for a registered health or social care professional to work with individuals who have a diagnosis of Autism and/or Learning Disability. The successful candidate will be responsible for ensuring safe and effective care for patients admitted to hospital, focusing on discharge planning from day one of admission.

You will work closely with hospitals, community services, and care providers to develop systems and processes to facilitate smooth responses to stakeholders. Your experience and knowledge will enable you to commission care packages in accordance with locally agreed protocols.

Responsibilities
  • Ensure patients receive safe and effective care with a focus on discharge planning from day one of admission.
  • Link with team leads working to avoid inappropriate hospital admissions for those with Autism and/or Learning Disability.
  • Responsible for admission avoidance and inpatient aspects of the Learning Disability and Autism Programme across Hampshire, Southampton, and The Isle of Wight.
Requirements
  • Registered Health or Social Care Professional with current registration with a professional body (i.e., NMC, HCPC, or SWE).
  • Experience of assessment of health and social care needs - care planning and coordination.
  • Knowledge and relevant experience working with individuals who are autistic and/or have learning disabilities and/or mental health issues.

The salary for this role is estimated to be around £43,000-£50,000 per annum, depending on experience.
