About the Role: As a Senior Learning Disability and Autism Practitioner, you will play a vital role in supporting individuals with learning disabilities and autism. You will work closely with hospitals, community services, and care providers to develop systems and processes that ensure smooth transitions and responses to stakeholders.

Key Responsibilities:

  • Lead the admission avoidance and inpatient aspects of the Learning Disability and Autism Programme across Hampshire, Southampton, and The Isle of Wight.
  • Collaborate with team leads to avoid inappropriate hospital admissions for those with a diagnosis of Autism and/or Learning Disability.
  • Support discharges from Acute hospitals, Assessment and Treatment units, and other inpatient units providing care for people with learning disabilities, Autism, or both.

Requirements:

  • Registered Health or Social Care Professional with current registration with a professional body (NMC, HCPC, or SWE), including AMHPs and mental health social workers or significant relevant experience.
  • Experience in assessment of health and social care needs, care planning, and coordination.
  • Knowledge and relevant experience working with individuals who are autistic and/or have learning disabilities and/or mental health issues.
