Job Opportunity: Experienced Care Manager

The successful candidate will work closely with the management team to deliver high-quality care services, ensuring seamless integration of mental health, learning disabilities, and autism provisions.

Key Responsibilities:

  • Develop and implement care plans tailored to meet individual needs
  • Foster strong relationships with service users, families, and multidisciplinary teams
  • Contribute to quality assurance initiatives, promoting continuous improvement

Requirements:

  • Proven experience as a Registered Care Manager or equivalent role
  • Strong understanding of mental health, learning disabilities, and autism principles
  • Effective communication and interpersonal skills
