Social Care Specialist

As a Social Care Specialist with CV-Library, you will be working in a fast-paced Adult Access Team at Cunard Building, providing assessment and reablement support to adults over 18. You will work alongside partner agencies in the Liverpool area and have support from a Team Manager or Senior Practitioner.
Key ResponsibilitiesAssessment and Reablement: Conduct thorough and detailed assessments, identify needs, and develop support plans to improve the quality of life for adults.
Care Act Assessments: Complete comprehensive assessments to determine eligibility for ongoing support within the community.
Mental Capacity Act Assessments: Carry out assessments to ensure individuals' rights are protected and their best interests are represented.
Reviews and Case Management: Participate in a Duty system to resolve cases not allocated, and conduct reviews to monitor progress and make adjustments as needed.
Requirements- Minimum 2 years' experience in adult social care
- Qualified Social Worker
- DBS
Salary: Estimated £43,780 per year (based on 37 hours/week)
