Key Responsibilities:- Provide assessment, treatment, and service coordination for clients with substance use concerns and/or concurrent mental health issues.
- Work collaboratively with an interdisciplinary team to ensure timely and responsive care for clients.
- Develop and implement individualized treatment plans to address clients' complex needs.
- Provide low-barrier outreach services to adults experiencing substance use concerns.
