Overview

Key Responsibilities

  • Develop and maintain effective communication pathways with hospital-based specialist teams to ensure seamless referrals and support.
  • Collaborate with acute services to identify and address the needs of repeat attendees, implementing strategies to prevent unnecessary hospital admissions.
  • Provide expert advice and guidance to clinical staff on the management of substance misuse, promoting best practices and reducing complications.
  • Lead the development of specific links with identified specialist acute services, ensuring in-reach to hospital wards and specialist clinics.
  • Assess, plan, implement, and evaluate care for a complex caseload of patients with substance misuse issues, both in hospital and community settings.
