About the Role

Change Grow Live is seeking a skilled Hospital Liaison Substance Misuse Nurse to join our multidisciplinary team. As a key member of our team, you will provide specialist clinical interventions to individuals who use our services, working closely with hospital staff to ensure seamless transitions between hospital and community care.

Key Responsibilities

  • Establish effective communication and referral pathways with hospital-based specialist teams, acting as a point of contact for acute referrals.
  • Participate in acute services to develop and maintain processes for targeting repeat attendees, implementing investigation systems to evaluate circumstances surrounding hospital admissions.
  • Provide support to acute hospital services to avoid unnecessary hospital admissions of individuals who can be supported and treated within community addiction services.
  • Develop specific links with identified specialist acute services, providing in-reach to hospital wards, including Gastroenterology/Hepatology, Orthopaedic-falls prevention services, and specialist clinics.
  • Provide advanced assessment, planning, implementation, and evaluation of care for a small, complex caseload of patients who misuse alcohol and/or drugs, while in a hospital setting or as part of discharge to community plans.
  • Offer expert advice and guidance to clinical staff on managing individuals who misuse alcohol and/or drugs, preventing further complications in the short and long term.
