Job Title: Support Worker - Community Care

We are seeking a dedicated and compassionate Support Worker to join our team in providing high-quality care and support to individuals in their own homes. As a Support Worker, you will play a vital role in enabling our service users to maintain their independence, dignity, and well-being.

Key Responsibilities:
  • Provide person-centered care and support to individuals with varying care needs
  • Assist with daily living tasks, such as personal care, meal preparation, and household chores
  • Encourage and support service users to engage in activities that promote their physical and emotional well-being
  • Work collaboratively with the care team to develop and implement personalized care plans
  • Maintain accurate records and reports as required
Requirements:
