We are seeking a highly skilled and dedicated Social Worker/AMHP to join our Community Mental Health Team. As a key member of the team, you will be responsible for providing high-quality social work interventions to individuals with complex mental health needs.
Key Responsibilities- Conduct comprehensive assessments and develop effective care plans to support service users in achieving their recovery goals.
- Work collaboratively with multidisciplinary teams to provide holistic care and support to individuals with severe and enduring mental health needs.
- Champion the Social Model within secondary mental health services, promoting a person-centred approach to care and support.
- Contribute to the development and implementation of policies and procedures that promote best practice in social work and mental health care.
To be successful in this role, you will possess:
- Excellent communication and interpersonal skills, with the ability to work effectively with individuals, families, and multidisciplinary teams.
- Strong assessment and intervention skills, with the ability to develop and implement effective care plans.
- Knowledge of mental health legislation and policy, with the ability to apply this knowledge in practice.
