Diabetes Specialist Nurse Opportunity

We are seeking a highly motivated and experienced Diabetes Specialist Nurse to join our community team at Hertfordshire Community NHS Trust.

The successful candidate will work closely with our partner organisations to provide an integrated service to adults with diabetes in South and West Hertfordshire.

Main Responsibilities:

  • Deliver high-quality care to patients with diabetes in community settings
  • Provide education and support to patients and their families
  • Work collaboratively with multidisciplinary teams to achieve optimal patient outcomes
  • Contribute to the development and implementation of diabetes services

Requirements:

  • Registered nurse with postgraduate experience in diabetes
  • Excellent communication and organisational skills
  • Ability to work independently and as part of a team
  • Good IT skills and ability to use electronic records
