Job Title: Support WorkerWe are seeking a Support Worker to join our team in Altrincham, Cheshire.
As a Support Worker, you will play a vital role in providing care and support to individuals with mental health needs, promoting their independence and well-being.
You will work collaboratively with our team to develop and implement person-centred care plans, ensuring that each individual receives tailored support to meet their unique needs.
Key Responsibilities:- Provide person-centred care and support to individuals with mental health needs
