Job Summary
Dorset HealthCare University NHS Foundation Trust is seeking a highly motivated and experienced Registered Mental Health Practitioner to join our Assertive Outreach West team.
The successful candidate will be responsible for providing person-centred targeted support to patients with severe and enduring mental illness, working closely with a multi-disciplinary team to promote social inclusion and recovery.
The role involves assessing and monitoring patients in their own homes, and requires a valid driving licence and use of a car.
The Assertive Outreach team covers pan-Dorset County, excluding Bournemouth, Poole, and Christchurch.
Main Duties
- Provide dedicated care coordination for a caseload of patients, using person-centred approaches to support their holistic recovery.
- Conduct regular clinics, including depot and Olanzapine administration, and physical health checks.
- Work closely with the multi-disciplinary team to promote engagement and recovery, and to advance clinical assessment skills.
The successful candidate will be a reliable, committed, and motivated individual with excellent communication and organisational skills, and a willingness to advance their own skill set through additional learning opportunities.
The post is for 37.5 hours per week, working Monday to Friday, 09:00-17:00 hours.
