About the Role

We are seeking a dedicated and motivated Mental Health Practitioner to join our Assertive Outreach team in Weymouth. As a key member of our multi-disciplinary team, you will work closely with clients who have severe and enduring mental illness, providing person-centred targeted support to aid their holistic recovery.

Main Responsibilities

  • Provide dedicated care coordination for a caseload of patients, applying philosophies of engagement and recovery in your work with service users.
  • Conduct regular clinics, depot, Olanzapine, and physical health assessments, with a focus on risk assessment and clinical skills development.
  • Work collaboratively with Consultant Psychiatrists, Psychologists, Nurse therapists, Mental Health Nurses, Support Time Recovery Workers, and Occupational Therapists to deliver comprehensive care.
  • Utilize your excellent communication, organisational, and prioritisation skills to manage your workload effectively.

What We Offer

We provide comprehensive structured management supervision, management support, and peer support, as well as opportunities for additional learning and skill development, such as NMP, ACP, and psychological therapies.

Requirements

  • Valid driving licence and use of a car for assessing and monitoring patients in their own homes.
  • Recent experience working in a mental health environment.
