Clinical Pharmacist

We are seeking a highly skilled Clinical Pharmacist to join our team at K2 Healthcare Ltd. As a Clinical Pharmacist, you will play a key role in developing and managing medicines optimisation services within our practice.
Key Responsibilities- Provide primary support to general practice staff with regards to prescription and medication queries.
- Help support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing.
- Provide clinical leadership on medicines optimisation and quality improvement.
- Manage some aspects of the quality and outcomes framework and enhanced services.
- Ensure that the practice integrates with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload.
- Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council.
- Two years post qualification experience.
- May hold or be working towards an independent prescribing qualification.
K2 Healthcare Ltd is a GP Federation covering 16 practices, serving over 130,000 patients across all sites. We have a team of 12 CPs working for us, including 3 senior CPs and 6 pharmacy technicians.
