About the Role
We are seeking a highly skilled Clinical Pharmacist to join our team at K2 Healthcare Ltd. As a Clinical Pharmacist, you will play a key role in enhancing and managing medicines optimization services in our practice.
Key Responsibilities
- Collaborate with our multidisciplinary team to optimize medication use and improve patient outcomes.
- Provide primary support to general practice staff on prescription and medication queries, manage repeat prescriptions, and handle acute prescription requests.
- Offer clinical leadership on medicines optimization and quality improvement, overseeing aspects of the quality and outcomes framework and enhanced services.
- Ensure integration with community and hospital pharmacy to optimize skill mix, enhance patient outcomes, improve healthcare access, and manage workload effectively.
- Work collaboratively with other Clinical Pharmacists in a supportive team environment.
Requirements
- Completed undergraduate degree in pharmacy and registered with the General Pharmaceutical Council.
- Two years post-qualification experience.
- Experience in Primary care preferred but not mandatory.
