Job summary

As a Clinical Pharmacist you will work with our current Clinical Pharmacist Team to further develop and manage medicines optimisation services within our practice. You will work in a small number of the Sleaford practice's as well as working from home on occasion. You will therefore have the opportunity to build up relationships with the primary care team. You will work together to manage repeat prescription authorisation and reauthorisation, acute prescription requests, management of transfer of care and systems for safer prescribing. You will have one day a week to concentrate on project work working with the 4 Pharmacy Technicians across the whole PCN with the support of our three senior CPs.

You will have the opportunity to develop areas of clinical interest in a patient facing role, develop or improve your prescribing skills and improve the management of patients with long term conditions. Mentoring and support will be provided.

You will be supported through your CPPE pathway and also your IP course if needed by our Team of 12 Pharmacists. You will not be working in any practice alone.

Main duties of the job

Please see the Job Description for full duties but you will will provide primary support to general practice staff with regards to prescription and medication queries. You will help support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patient in the GP practice(s).

You will provide clinical leadership on medicines optimisation and quality improvement and manage some aspects of the quality and outcomes framework and enhanced services.

You will ensure that the practice integrates with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload. The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice.

You will work with the other CPs in our large team being fully supported throughout.

About us

We are a GP Federation covering 16 practices. 10 in Grantham and surrounding villages and 6 in Sleaford. We cover 130,000+ patients across all sites. We have a team of 12 CPs working for us including 3 senior CPs and 6 pharmacy technicians. Each of the practice's has at least one CP in post to help support and mentor if required.

Our CP Team are all at different stages of completing their CPPE / IP course and are happy to speak to you regards their experience with K2 Healthcare.

Job responsibilities

The post holder is a pharmacist, who acts within their professional boundaries, supporting and working alongside the Pharmacist and Dispensary team in the Practice, who will help develop, manage and mentor them.

The post holder will work as part of a multi-disciplinary team in a patient-facing role. The post holder will take responsibility for areas of chronic disease management within the practice and undertake clinical medication reviews to proactively manage patients with complex polypharmacy.

The post holder will provide primary support to general practice staff with regards to prescription and medication queries. They will help support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patient in the GP practice (s).

The post holder will be supported to develop their role to become a nonmedical prescriber if required.

Person Specification

Qualifications

Essential

Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council

Desirable

May hold or be working towards an independent prescribing qualification. Recognises priorities when problem-solving and identifies deviations from normal pattern and is able to refer to seniors or GPs when appropriate May have completed CPPE Pathway
