About the Position

This role is part of a multi-disciplinary Mental Health and Addictions Team providing a comprehensive array of services within a psycho-social rehabilitation framework to adults with serious and persistent mental illness and/or within a harm reduction framework for those with substance use problems/disorders and who experience severe challenges in daily living as a result.

The successful candidate will provide direct care, including supportive counseling, risk assessment, and crisis intervention for individuals or groups who experience severe challenges in daily living as a result of their mental illness and/or substance use problems/disorders.

They will also participate in the development, coordination, implementation, and evaluation of a treatment plan via an outreach methodology for assigned clients.

Additionally, the candidate will participate in team clinical reviews and consultation, psychiatric reviews, and the monitoring of clients, and perform other related duties as required.
