Job Overview

Oxford Health NHS Foundation Trust is seeking an exceptional Clinical Triage Specialist to join our Single Point of Access (SPA) team. As a highly motivated and skilled practitioner, you will play a crucial role in delivering high-quality triage services, enabling members of the public, GPs, and other healthcare professionals to access community health services effectively.

Main Duties of the Job

  1. Deliver and maintain high-quality triage services, utilizing enhanced assessment, problem-solving, and decision-making skills to identify patients' needs and appropriate destinations for care, including management of clinical risk.
  2. Referrals will be made to and from the urgent community response, crisis and social care, community hospitals, and a range of community/specialist services.
  3. Work collaboratively with a supportive team to deliver high-quality patient care, handling complex and diverse referrals.
  4. Participate in ongoing training and professional development to enhance your skills and knowledge.
