Job Overview

An exciting opportunity has arisen for a highly motivated and experienced healthcare professional to join our busy but rewarding Single Point of Access (SPA) team at Oxford Health NHS Foundation Trust.

We are seeking an exceptional clinician who can work autonomously and within a team to deliver high-quality patient care and outcomes in the community.

Main Duties of the Job

The successful candidate will be responsible for delivering and maintaining a high standard of triage, using enhanced assessment, problem-solving, and decision-making skills to identify patient needs and appropriate destinations for care.

This includes referrals to and from urgent community response, crisis, and social care services, as well as community hospitals and specialist services such as district nursing, community therapy, falls service, and palliative care.

The ideal candidate will have experience of community health services and be able to demonstrate the ability to initiate and manage work effectively, with a commitment to delivering positive outcomes for patients.

Person Specification

The successful candidate will possess:

  • A registered health professional qualification
  • Education to degree level or equivalent
  • Experience of working with older people
  • Ability to communicate with those who may have a barrier to understanding

Desirable criteria include post-registration courses in relevant areas and recent/current community knowledge and experience.
