Job Description

We are seeking a dedicated and compassionate Practitioner in Adult Care to join our team in North Lanarkshire.

This role involves working with individuals who require support with daily living tasks, enabling them to maintain their independence and quality of life.

As a Practitioner in Adult Care, you will be responsible for providing holistic support, addressing physical, emotional, and social needs.

You will also contribute to the development and implementation of personalized care plans, collaborating with colleagues and stakeholders to ensure seamless service delivery.

Responsibilities
  • Deliver person-centred care, addressing physical, emotional, and social needs
  • Develop and implement personalized care plans, in collaboration with multidisciplinary teams
  • Maintain accurate records and reports, ensuring timely communication with colleagues and supervisors
  • Contribute to ongoing quality improvement initiatives, identifying best practices and areas for development
