About the Role

We are seeking a skilled and compassionate Support Worker to join our team in providing high-quality care and support to individuals living within their homes in the local community.

As a key member of our team, you will be responsible for implementing Personal Plans to meet individual outcomes, promoting independence, and supporting service users to maintain their safety and well-being in their own environment.

Key Responsibilities:
  • Provide front-line care and support to individuals living within their homes in the local community, ensuring a person-centred approach is provided as part of this practice.
  • Promote independence to support individuals to achieve their identified personal outcomes.
  • Work alongside colleagues and other professionals to promote well-being, positive risk-taking, and community presence.
  • Complete relevant documentation in line with required policies.
  • Engage and participate in team meetings/training and engagement sessions.
