About the Role

We are seeking a highly skilled and dedicated Community Mental Health Nurse to join our dynamic and supportive team. As a key member of our Older Adult Mental Health Team, you will play a vital role in providing high-quality care to patients with functional and organic mental health needs.

Key Responsibilities
  • Manage a caseload of patients with complex mental health needs, using evidence-based and patient-centered principles to assess, plan, implement, and evaluate interventions.
  • Work collaboratively with patients, carers, and other local agencies to identify therapeutic goals and develop individualized care plans.
  • Plan and implement patient-centered individual and group interventions to achieve therapeutic goals.
  • Maintain accurate and timely clinical records, ensuring seamless communication with patients, carers, and other healthcare professionals.
  • Provide leadership and supervision to junior staff, promoting a culture of excellence and continuous learning.
