Key Responsibilities

  • Develop and implement personalized care plans, focusing on enhancing life skills and promoting community engagement.
  • Support individuals with daily aspects of living, including medication administration, cooking skills, and accessing the community.
  • Collaborate with colleagues to ensure seamless support and care, maintaining a positive and inclusive team environment.
  • Complete relevant paperwork to a high standard, ensuring accurate and timely documentation.
  • Support individuals to budget finances, promoting financial independence and responsibility.
  • Provide companionship and emotional support, engaging individuals in activities that enhance their life skills and promote independence.
  • Drive individuals to appointments and activities, utilizing a manual driving license.
