About the Role

We are seeking a highly skilled and experienced Mental Health Practitioner to join our General Practice integrated mental health service (GPimhs) in Epsom PCN.

This is a Full-Time (37.5 Hrs - Monday to Friday 9am to 5pm) role, working from the GP Practices within Epsom PCN and Horizon SABP Hub in Epsom.

The successful candidate will take a key role in developing and shaping a major new and transformative approach to providing mental health care within the community.

They will be responsible for working closely with General Practitioners and the primary care team to provide assessments, brief interventions, and care navigation for people with significant mental health issues living in the locality.

The successful candidate will need to work closely with local mental health services to support an easy in and out approach between secondary and primary care.

We work closely with other NHS and voluntary sector organisations who provide services and support people who use services and carers.

Key Responsibilities
  • Provide assessments, brief interventions, and care navigation for people with significant mental health issues living in the locality.
  • Work closely with General Practitioners and the primary care team to develop and implement effective mental health care plans.
  • Collaborate with local mental health services to support an easy in and out approach between secondary and primary care.
  • Develop and maintain strong relationships with local community groups and organisations to promote mental health and wellbeing.
Requirements
  • Qualification in Mental Health Nursing, Occupational Therapy, or Social Work at Degree level or equivalent Professional Registration.
  • Professional registration such as NMC/HCPC.
  • Experience of providing a range of evidence-based interventions to people with a variety of mental health problems and serious mental illness.
  • Ability to work independently and as part of a team to deliver high-quality mental health care services.
