We are seeking an Advanced Practitioner in Community Mental Health to join our team at South West London and St George's Mental Health NHS Trust. In this role, you will be responsible for delivering care coordination and supporting structured clinical management within our community setting.

About the Role

You will work closely with our Clinical Psychologist to develop and maintain the necessary skills for working with clients with complex emotional needs. This is an exciting opportunity to join our team and contribute to the delivery of high-quality care for individuals with mental health issues.

Key Responsibilities
  • To develop and implement care plans that meet the individual needs of clients and carers.
  • To participate in clinical and managerial supervision.
  • To work collaboratively with other healthcare professionals to provide comprehensive assessments, risk management, and crisis intervention services.
Requirements
  • Current professional registration (Nurse, Occupational Therapist, Social Worker, Clinical/ Counselling Psychologist) OR Clinical Associate Psychologist OR undergraduate degree plus post-graduate practitioner diploma in psycho-social interventions or a related field.
  • Experience of reflective practice and evidence-based interventions for complex emotional needs.
