About the Role

We are seeking an experienced and compassionate Support Worker to join our Independent Living Services Team. As a Support Co-ordinator, you will play a vital role in providing bespoke support packages to young people with complex needs, empowering them to develop their independence skills and achieve their aspirations.

Key Responsibilities
  • Provide positive support and reinforcement to young people, promoting their independence and well-being.
  • Establish and maintain strong relationships with service users, their families, and other stakeholders.
  • Work collaboratively with the team to ensure seamless communication and effective handovers.
  • Accurately document notes and maintain accurate records on our computer system.
  • Participate in training and development opportunities to enhance your skills and knowledge.

Requirements
